IT and Facilities Request

Whatever you do, work at it with all your heart, as working for the Lord, not for men, since you know you will receive an inheritance from the Lord as a reward. Colossians 3:23–24

A new work-order request system for all of your IT and Facilities needs has been implemented. It represents the sole source for reporting all non-emergency requests for the Facilities and IT departments.

 IT and FACILITIES WORK ORDER REQUEST

The form asks for the following information. Be as detailed as possible.

  1. Contact information: Name, email, contact phone number (optional)

  2. Location: This is the location for the work to be performed. Drop downs should include most areas, but get as close as possible if you don’t find what you are looking for and provide additional location details in the work description later. 

  3. Priority: Options will be high, medium or low priority, with Emergency and Project as additional options. Please CALL the Front Desk receptionist for emergencies like active leaks or items that pose a threat to people/property. 

  4. Problem Code: A category for the work, follow the drop down as closely as possible, and then provide additional details in the work description if needed. 

  5. Work Description: An open text box for you to write out the work you are requesting to be completed. Please be as detailed as possible and provide any additional information that will help us complete the work as efficiently as possible. 

  6. Last Step: Click on save new record at the top left of the page, and a work request will be generated. You will receive a copy when it is approved.

 

Note: (a) Use this system for all requests. Do not make requests to individual members of the department. (b) Choose the most accurate “problem code” and then be detailed in the work description. Include available times for the work to be completed, especially if in a classroom with limited availability.  (c) Include your email address in the contact information. (d) Complete the survey when you receive it. (e) Please do not assume we know of an issue. While we do complete regular inspections, items are missed. (f) You will be contacted in the event that there will be a delay.
